A patient journey process map illustrates the journey that an individual goes through from the moment that the health issue was first felt or recognized, up until monitoring and follow-up care.

The map is broken down into four core phases: Pre-Diagnosis, Diagnosis, Treatment and Monitoring. Each phase has steps that often happen during that phase. The map is a generic example that can apply to patients experiencing either simple or complex illnesses, but it can be customized to properly capture the journey of patients you want to document the experience of.
