Root Cause Analysis - Ishikawa + 5 Whys

This root cause analysis combines the power of the Ishikawa diagram (fishbone diagram) to document all the potential causes of each area broken up in each category of the 6Ms in Six Sigma, which are: Manpower(people), Machinery (technology), Materials, Methods, Measures, and Mother Nature (environment). The next step is to diagnose the root cause using the 5 whys for each cause. This way, the user can understand the issues and identify the underlying causes to avoid them from recurring.
