This Failure Mode Effects Analysis (FMEA) workshop template is a structured method to identify and address potential failures that have a severe impact to the final output to your ultimate process customer.

Potential process failures are prioritized according to (1) how severe the effect is to the customer, (2) how often the cause or failure occurs and (3) how detectable the cause or failure is.
