Effective Feedback Course

This template was developed to use as a 'check-in' with learners to receive feedback on a course. Learners need to answer questions by putting an emoji by the response they agree with most. Then, they are asked four open-ended questions in order to gain further feedback on the course and how to potentially improve it in the future.
