The 5 Whys Root Cause Analysis is a technique that aims to solve problems by determining the root cause and asking the question "Why?" five times. It was developed by Sakichi Toyoda, the founder of Toyota.

This process starts by identifying the main issue and the immediate cause of the issues. For each cause, you are encouraged to reflect further about the deeper cause until you've determined the root cause of the issue. This process can be done as an individual or as a team activity.
