Six Sigma DMAIC Root Cause Analysis
Brief
The Six Sigma DMAIC Root Cause Analysis template is a structured, five-phase problem-solving framework organized left to right across five columns: Define, Measure, Analyze, Improve, and Control. Each phase contains guiding questions at the top and a dedicated workspace below for capturing findings. The template enforces a data-driven sequence—users must not skip to Improve before Analyze is verified with data.

How to Use It
Work left to right through the five phases:
Define – State the problem, identify the customer, set the goal and scope.
Measure – Establish a baseline metric and collect current performance data.
Analyze – Identify root causes using 5 Whys or a fishbone diagram; verify causes with data. (A 5 Whys or fishbone frame can be attached to this phase.)
Improve – Develop fixes that target verified root causes; pilot before full implementation.
Control – Standardize the solution, monitor results, and define a response plan to sustain the gain.
Example
A product team tackling software defects escaping to production:
Define: Too many defects reach end users; goal is to cut escaped defects by 50% by end of Q4; scope is the checkout and payments squad.
Measure: Baseline of 14 escaped defects per month (May–Jul); 60% traced to the checkout module; support spending ~90 hours/month on defect tickets.
Analyze: 5 Whys on the top 10 defects revealed no automated regression suite and rushed pre-release reviews; 8 of 10 defects would have been caught by regression tests.
Improve: Built a 120-test automated regression suite; added a PR review checklist and a 24-hour release freeze; piloted with the checkout squad in September.
Control: Weekly defect dashboard; alert threshold set at 5 escaped defects/month; regression suite mandated in CI for all squads from October. Result: escaped defects dropped from 14 to 6 per month after the pilot.
