How to use it
Start with the Review context box to agree the topic, scope, stakeholders, and outcome needed.
Then complete each row from left to right:
What is happening now and what should happen instead?
Where does the issue occur?
When does it need to change?
Who is involved, accountable, or missing?
How should the process, resource, or capability improve?
Use the discussion to identify one or more priority gaps. Finish with the Summary Gap Analysis to assign an owner, target date, success measure, and review date.
By the end, you will have a shared view of the current situation, desired future, priority gaps, practical actions, accountable owners, and measures for tracking progress.
