What it is for:A feedback capture grid is a simple four-part sheet for reviewing a real test: what worked, what failed, what questions remain and what new ideas came up, with evidence beside each entry. You walk away with one shared, evidence-backed record of a real test: what worked, what failed, what you still do not know, and the ideas worth trying.
Who it helps:Teams who have just run a usability test, prototype session or pilot and want one shared, honest record of what happened.
When to use it
Right after a usability test, prototype session, pilot or concierge test, while memories are fresh.
When several people observed the same test and you want one shared record.
When feedback is mixed and the team risks hearing only what it hoped for.
How to use it
Draw four boxes: Worked, Failed, Question, Idea, and name the test at the top.
Everyone writes observations alone for five minutes, one per note.
Place notes in Worked and Failed, each with its evidence.
Capture open questions, then collect ideas last so they do not steer the review.
Pick the next question to test and share the grid the same day.
Tip:Hold the session within 24 hours of the test, before memories turn into stories.
