How to Use 4-Prong Fishbone Diagram

  • Define your problem statement clearly in the fish head and align your team on scope

  • Brainstorm causes category by category (Surroundings (environment), Suppliers (external inputs), Systems (processes/tools), Skill (people/knowledge))

  • Add sub-causes to each bone by asking "What leads to this?" to move from symptoms to root causes

  • Apply 5 Whys to top items, validate with data, then use dot voting to prioritize 3–5 high-leverage roots

  • Convert priority causes into SMART actions with owners and measures, then test and iterate
