The 5 Whys - Root Cause Analysis

Report

What is it?

Five whys (5 whys) is a problem-solving method that explores the underlying cause-and-effect of particular problems. The primary goal is to determine the root cause of a defect or a problem by successively asking the question “Why?”.

How to use it?

1) Define the problem

2) Let your team describe the problem from different perspectives

3) Start by going top-down and ask "why". Start from the first Blue ring and go all the way down to the fith core layer which hopefully will be the root cause of the problem.
